HealthCare Support is seeking a Field Nurse Practitioner or Physician Assistant to join a rapidly growing healthcare provider serving patients in Casa Grande, AZ . This is a unique opportunity to deliver high-quality, patient-centered care directly in the home—focusing on Chronic Kidney Disease and primary care needs. Conduct comprehensive in-home patient visits, including history and physical exams Develop and implement individualized treatment plans Order and interpret diagnostic tests and lab work Prescribe medications as appropriate Provide ongoing patient education and chronic disease management Complete accurate and timely documentation of services Collaborate with interdisciplinary care teams and participate in routine meetings Utilize telehealth to support patient care when appropriate Benefits for the
