The Podiatry Nurse Practitioner (NP) would be authorized to perform routine provider duties on a regular and repetitive basis within the designation NP scope of practice. These duties shall be performed independently, and in accordance with current Arizona Foot & Ankle Specialists (AzFAS) directives. Work Schedule:
This position is either Part-Time or Full-Time. The AzFAS Office is open Monday through Friday, 8:00 am to 5:00 pm, or when the last patient has left the office, with a break for lunch. Nurse Practitioner in this position is expected to arrive at AzFAS office at 7:45 am and remain until to 5:00 pm or when the last patient has left the office. General Requirements:

References Upon acceptance of NP position and execution of employment contract, the NP shall supply the office with a copy of all required supporting documents required by the government such as: 1. Completed new Employee AzFAS Packet/Demographics 2. Completed new Employee A4 Arizona form. 3. Completed new Employee W-4 Federal form. 4. Minimal Qualifications/Educational Requirements:
1. Possess a master's degree from a program accredited by the NLNAC or CCNE; national certification as an Adult Nurse Practitioner (ANP), Family Nurse Practitioner (FNP), or Acute Care Nurse Practitioner; MSN or DNP. 2. U.S.
citizenship or possess a valid/current work visa. 3. Graduate of a school of professional nursing approved by the appropriate State- accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. 4. Must be a current, full, active, and unrestricted Arizona licensed Nurse Practitioner with current board certification and must keep that certification current, performing all required tasks. 5. Must be, or become, member of a nationally recognized nurse practitioner association. 6. Is responsible for and must maintain all CME and work hour requirements. 7. Current DEA license. 8. Must have an NPI provider number. 9. Current Arizona RN license. 10. Must maintain current BLS certification. 11. RN First Assistant certification is not currently, required but may be so in the future. Responsibilities/essential functions:
This position requires the ability to perform various duties and essential functions. These responsibilities include, but are not limited to the following: The NP must be teachable, have a desire to learn new things, and perfect known things, to help provide the best quality of care to all our patients. Understand the limits and abilities of the NP's knowledge base and skill level and seek advice and knowledge from the supervising physician whenever needed and timely. Understand the limits of podiatry scope of practice in the state of Arizona and while employed with AzFAS the NP must stay within them. Treat all patients with kindness and compassion and provide the best quality and evidenced based treatments for the individual conditions, relying on and following proven AzFAS Practice Protocols. Demonstrates responsibility and accountability for own professional practice. Help other administrative and/or billing staff with any credentialing requirements needed for insurance companies, insurance panels, ACO's, Hospitals, MSO's, etc. Perform initial histories and physical examinations on new patients and subsequent history and physical examinations on existing patients, and the same on any new or established patients in the hospitals were credentialed. All initial history and physical examinations will be reviewed by the supervising physician, who will note concurrence or addendum, as indicated. Assist and mentor the Medical Assistants, regularly, and even Medical Students and/or Podiatry Residents when applicable, in the delivery of care to assigned patients. Provide ongoing outpatient care for assigned patients, including, but not limited to: ordering, performing, and interpreting laboratory, radiographic and other diagnostic tests, and implementing appropriate treatments. Screen patients to determine any need for further health care evaluations and/or treatments needed for any additional or new conditions noted on exam or discussions with patient. Prescribe and dispense medication in accordance with Arizona Board of Nursing Practice Act and AzFAS treatment protocol policies. Arizona Prescribing/dispensing with a DEA:
NP may prescribe non-controlled drugs. Schedules II controlled substance may be prescribed once with no refills of the prescription; Schedules III and IV controlled substance may be prescribed a maximum of five refills in six months; and Schedule V controlled substance may prescribe refills for a maximum of one year. DEA registration required. Refer to and consult with appropriate health care professionals in a timely manner. Enter inpatient admission orders including post-operative orders or outpatient preoperative surgical orders for DPM's. Obtain informed consent from patients preoperatively per AzFAS policy and procedures. The NP may obtain consent for those procedure(s) that the NP has an understanding of the planned procedure(s), the reasons for the procedure(s) and expected outcomes. The NP must have detailed knowledge base of the procedure, risks, benefits, and alternative treatments. Order tests and consults, as appropriate, for each patient under their direction. Organize and support the AzFAS office and other staff, assist the supervising physician(s) in evaluation and management of patients, provide patient education regarding physical limitations, provide diabetes, weight loss, and/or smoking cessation education, perform pre-operative examinations, be available for any in- office emergencies, provide diagnosis specific patient education and provide written education were applicable, complete any return to school or work slip letter, complete insurance paperwork, and other services as appropriate for the daily management of the office. Initiate and expedite requests for consultations and scheduling special tests and studies, including, but not limited to SudoScan Neurological testing, PAD Net Vascular testing, biopsies, NCV/EMG's, Diagnostic Ultrasounds, and MRI/CT scans. Identify patients suitable for pre-op screening. Coordinate patient information including H&P's, test results, etc. Provide patient education regarding the surgical procedure, pre- and post-operative course, and other topics, as appropriate. Assist in overall post-operative care, post-fracture care, and wound care, and following the action plan or treatment protocols, and evaluating the progression or course of healing, and if any negative changes occur, then alert the corresponding physician as to the changes noted. Receive and correlate serial lab data. Make rounds to observe and record patient's medical progress, updating and summarizing medical records, reviewing lab data, ordering additional labs or changing orders when appropriate, and notifying the responsible physician of significant changes in the patient's condition. Assist in hospital rounds, dressing changes, and patient assessments. Write progress notes, new orders, discharge orders and instructions. Dictate or write discharge summaries when applicable. Assist other providers or staff by providing information necessary for data collection. Participate on committees and/or organize, implement, and manage selected roles within the office when assigned. Provide general coordination and support of the other providers at AzFAS. Adhere to Clinical Practice Guidelines and Prevention Disease Indices. Routine incision and drainage of infected spaces, provide wound care and debridement, and sharp and enzymatic wound debridement with supervision, until full competency is demonstrated Perform duties as assigned, such as administrative duties, education of medical residents and allied health care students, participation in clinical research on patients with wounds (including debridement) and draw blood specimens for testing and performing other comparable laboratory procedures, as needed. Order radiographs of the foot, ankle, or lower leg. Radiographs will be read by in house doctors or the supervising physician. Discuss findings of radiographs with patients as needed. Evaluate and perform needed Diabetic foot care, Diabetic foot examinations, At Risk Foot Care such as nail debridement or trimming as needed, sharp debridement of corns, callouses, or distal clavi affecting the patient. Orthotic Management including a thorough understanding as to the reasons for the orthotic, the fit of the orthotic, any needed modifications of the orthotic, time for refurbishment and or replacement of the orthotic, prescription and ordering of the orthotic, dispensing of the orthotic including break-in instructions, discussions with patient as to how the orthotic fits in their current shoe and any shoe gear changes that are necessary, casting of the orthotic, and billing for the orthotics. Wound Care including evaluation of the status of the wound, progression or worsening, understanding current course of wound care and when that care needs to be altered or changed, wound dressing changes, Unna boots, multi-layer compression dressings/garments. Injections:
soft tissue, joint, trigger point, biologics, Amniotic tissue injections or membrane graft placement. Must have a thorough understanding and knowledge base of how and when to use the office-based ancillary's, including but not limited to shoe gear/sandals, prefabricated insoles/orthotics, custom orthotics, padding's and/or strappings, radiographs, diagnostic ultrasounds, SudoScan neurological testing, PAD vascular testing, AFO's (ankle foot orthotics), Moore Balance Braces for Fall Risk Prevention, Laser toenail therapy, Laser inflammation therapy, Cryotherapy, Laser Wart therapy, EPAT/shockwave therapy, protective braces such as surgical shoes, CAM Walkers, and crutches, miscellaneous creams, lotions, wound care supplies, antifungal nail polish, other digital and foot aids. Thorough understanding and knowledge base of how to properly select the correct diagnostic codes (ICD 10) and input into the EMR program for the selected patient. Thorough understanding and knowledge base of how to bill for office visits, office procedures, office-based testing and labs, and any DME, medical supplies or other devices the patient needs for home use. Thorough understanding and working knowledge of the office EMR program, maintaining medical records for all patient seen, including completion of the progress notes in a timely fashion, which all notes are expected to be completed by the end of the day of the visit, but no longer than 72 hours. Completion of all prior authorization request, work status reporting for individual patients, any peer-to-peer consultations, prescription forms, observation requests, or other necessary paperwork in a timely manner. Help with getting the office-based surgery suite/center up and functional and then maintaining all licensing and other certifications or requirements to keep it in compliance. Additional (Human Resources) Duties include but are not limited to: Maintain privacy and confidentiality of all employee personal information and rates of pay. o Assist administration with employee verification, documentation, testing, various filing. Assist in the interviewing and full hiring process, for all job applicants, and new hires from date of hire to separation from AzFAS, LLC. In conjunction with CEO, and Lead Medical Assistant, respond to breaches of office policy appropriately, including necessary documentation and discipline as needed. Complete employee evaluations annually. Ensure that all new hires complete necessary employment forms, receive practice policies and procedures information, and that all appropriate forms are signed, dated, and stored in the employee's personnel file. o Review and ensure that personnel files of all employees are up to date every 90 days. o Maintain files on all independent contractors under contract with the practice, including insurance information, HIPAA business agreements, etc. Maintain and update the Employee Handbook as needed. Notify employees when a change has been made to the handbook and provide updated copies of the entire handbook or individual documents to all employees as needed.
