• + Clinic-based on-site at Banner Health Center, 17900 N Porter Rd, Maricopa, AZ 85138 + Provide telehealth visits to established patients and manage clinical inbox for care coordination + Flexible scheduling: 4x10s or 5x8s shifts + Medical Assistants, Front Office, and leadership support + Support of an integrated system including specialists, urgent care, and more!

• Minimum Requirements:

• + DEA with prescriptive authority required + FNP or PA-C designation with AZ license eligibility + Minimum (1) year of experience as an FNP or PA in a primary care setting + Patient-focused, friendly, and willing to collaborate in a team environment
