Job Description

We are seeking a full-time Adult-Gerontology Nurse Practitioner in Mesa, AZ to deliver high-quality care to adult and older adult patients. The NP will perform comprehensive assessments, establish diagnoses, develop and implement treatment plans, prescribe as appropriate, and work collaboratively with interdisciplinary teams to achieve optimal patient outcomes.

Qualifications:
Master's or Doctoral degree in Nursing with an Adult-Gerontology Nurse Practitioner focus Current Arizona NP licensure Certification as an Adult-Gerontology Nurse Practitioner Clinical experience in adult and geriatric care
Key Responsibilities:
Conduct thorough patient assessments and diagnostic evaluations Provide primary care services including acute and chronic disease management Develop individualized treatment plans and prescribe medications within scope of practice Communicate and coordinate care with physicians, specialists, and other team members Deliver patient-centered education on disease prevention, self-care, and medication management Address geriatric-specific needs such as polypharmacy management, mobility, cognition, and functional assessment Maintain accurate, timely documentation in the medical record Participate in community outreach and advocate for older adult health needs This role requires strong clinical judgment, excellent communication skills, and a commitment to compassionate, evidence-based care for adult and geriatric populations.
