We have an opening in our Mesa location. The Nurse Practitioner reports to the President of Valley Sleep Center. This position practices in either Adult or Family Medicine. The Nurse Practitioner consults with both new patients and established patients who have sleep disorders including disordered breathing in sleep, insomnia, parasomnia and disordered movement in sleep. The Nurse Practitioner has a close working relationship with the President, the Director of Patient Services, the Director of Technical Services and the Medical Director to ensure patient care standards are met throughout the Valley Sleep Center locations. In all interactions, the Nurse Practitioner ensures the first and lasting impression of Valley Sleep Center is as a provider of excellence in patient care.
Primary/Essential Job Functions:
Examine patients and establish a medical diagnosis by reviewing client history including sleep assessment, complaints and physical examination. Provide and coordinate care for patients throughout the continuum of care to assure that care is timely, appropriate, of high quality and cost effective. Order and interpret laboratory, and diagnostic tests and perform those tests that the RNP is qualified to perform. Prescribe, order, administer and dispense therapeutic measures including pharmacologic agents as authorized by the State of Arizona, and non-pharmacological interventions including, but not limited to, durable medical equipment. Identify, develop, implement, and evaluate a plan of care for a patient to promote, maintain, and restore health. For a patient who requires the services of the Sleep Center facility, to order appropriate testing, evaluation of results and develop treatment plan with patient. Perform therapeutic procedures that the RNP is qualified to perform. Delegate therapeutic measures to qualified administrative personnel including medical assistants. Case management of sleep disorder workups to ensure the patients get routed to the proper tests and prescribed equipment and follow-up, making use of the assistance of administrative support person(s) to help with record keeping and case tracking and non-clinical communications. Assess patient/family/staff education needs and develops, implement and re-evaluate education strategies. Customize teaching methods based on age/development/ ethnic background of recipient. Act as patient advocate with multidisciplinary coordination with referring providers, DME, referrals, collaborating physicians, medical insurance and community resources to improve health care delivery. Perform additional acts that the RNP is qualified to perform and that are generally recognized as being within the role and population focus of certification. Nurse Practitioner upholds standards of ethics, as defined by his/her affiliated professional organizations. The Nurse practitioner places patient care above personal gain. The Nurse Practitioner cooperates with and listens to the opinions of others. Nurse Practitioner treats patients, colleagues and staff with respect and courtesy. Maintain Collaborative relationship for consultation or referral with one or more licensed physicians on an as-needed basis. Direct or on-site supervision of the activities of a RNP by the collaborating physician is not required. Direct collaboration relationship is needed with the Medical Director of the center associated with the work in question. Patient encounters with sleep consult referrals will be reviewed using screening methodologies as directed by the Medical Director. Management and problem solving of Sleep Medicine processes and patient flows, which may include taking the lead in negotiations and discussions devising procedures with both internal (Medical Director, sleep technicians, RTs, DME) as well as external parties. An RNP shall refer a patient to a physician or another health care provider if the referral will protect the health and welfare of the patient. A consultation with a physician and other health care providers may be necessary if a situation or condition occurs in a patient that is beyond the RNP's knowledge and experience and within the limits of the population focus of certification.
Skills and Competencies:
Comply with applicable laws, regulations, guidelines and standards regarding safety and infection control issues Maintain current CPR or BCLS certification Comply with the Arizona Board of Nursing Standards of Conduct Compliant with HIPAA policies and guidelines Demonstrate appropriate social skills Demonstrate the ability to analyze complex situations and apply policy Demonstrate excellent interpersonal, verbal and written communication skills Demonstrate proficiency with computers and electronic health records Work and contribute successfully within a diverse work and business environment Education and/or
Experience:
Non- restricted state license for RN and RNP including prescribing privileges and DEA number. Hold RNP national certification from a national certifying body recognized by Arizona State Board of Nursing. Preferred candidates should have experience in sleep medicine or 3 plus years in chronic disease management.
CME/ CEU
provider requirements set by the AASM for accredited outpatient sleep labs. CME/CEU requirements set by Arizona State Board of Nursing and those required by RNP certifying body.
