Job Description

Position Overview:
We are seeking a compassionate and dedicated Mobile Nurse Practitioner to join our team. In this role, you will provide comprehensive primary care and transitional care services to patients in the comforts of their homes. While you will be primarily seeing patients in their homes, you will also conduct telehealth visits and in-clinic appointments as needed. A key requirement for this role will be complex wound care. Looking to hire in all parts of the valley.

Responsibilities Conduct comprehensive physical assessments in patients' homes Travel within a reasonable service area Diagnose and treat common acute illnesses and injuries Conduct complex wound care Manage chronic health conditions Order, interpret, and document diagnostic tests Prescribe medications and treatments as appropriate Submit accurate and timely progress notes Educate patients and families on health maintenance and disease prevention Collaborate with physicians and other healthcare professionals Provide in-person and virtual face-to-face visits for hospice patients Role Highlights Flexible, mobile-first position with occasional in-clinic days in Scottsdale Service areas include West Valley, Paradise Valley, Scottsdale, and North Scottsdale Choice of compensation model: revenue split or pay-per-visit Qualifications Current certification as a Family Nurse Practitioner (FNP) or Physician Assistant (PA) Valid Arizona license to practice and active DEA license Strong clinical knowledge of primary care medicine Excellent communication and interpersonal skills Compassionate, patient-centered approach to care Ability to work independently and collaboratively within a care team Proficiency with electronic medical records (EMR) systems and internet-based tools Experience & Licensure Minimum of 4 years of experience in medical documentation (required) Active RN license (required)
