Circle the City is a rapidly growing non-profit community health agency dedicated to providing high quality, holistic healthcare to people experiencing homelessness in Arizona. Our homeless continuum of care is one of a kind in the State of Arizona, and one of few similar models across the nation.

Circle the City patients benefit from several different levels of care offered within our organization: The sickest of our patients are offered the opportunity to rest and heal in our two state-of-the-art 50-bed medical respite centers. Additionally, patients suffering from mental illness may receive psychiatric consultation at any of our service sites, or many benefit from the more intensive, community-based services offered by our assertive community treatment team. Circle the City will also soon maximize our community outreach efforts via a 2-exam room mobile clinic, and street based medicine program, slated to launch later this year. Circle the City strives to meet each of our clients "where they are," without judgement or bias. The Circle the City Nurse Practitioner provides high quality holistic health care as an integral part of a dynamic interdisciplinary team. Care is provided in multiple settings including the two innovative 50-bed Medical Respite Centers, the Parsons Family Health Center, the Downtown Family Medical Center and the mobile medical unit, and street outreach sites.

Essential Duties:

Duties include, but are not limited to: Essential Duties Duties include, but are not limited to: Primary Care Responsibilities (Majority of Role) Provide high-quality, patient-centered primary care to adults experiencing homelessness, including diagnosing and managing acute and chronic medical conditions. Assess patient health through interviews, examinations, and review of medical histories. Order and interpret diagnostic tests, such as x-rays, EKGs, and laboratory studies. Develop and implement patient treatment plans, ensuring continuity of care and smooth care transitions. Instruct and counsel patients on therapeutic regimens, preventive care, and health maintenance. Collaborate and consult with supervising physicians and interdisciplinary team members as needed. Document patient care services in the electronic health record in a timely and accurate manner. Maintain compliance with infection control policies, safety standards, and regulatory requirements. Work collaboratively and respectfully as a core member of the interdisciplinary healthcare team. Memory Clinic Responsibilities (One Day per Week with Potential to Expand) Serve as the medical provider for Circle the City's Memory Clinic, conducting cognitive evaluations and ongoing management of dementia and related conditions. Work collaboratively with an integrated team consisting of behavioral health, social workers and community partners to address medical, mental health, and social needs. Provide trauma-informed, person-centered care to seniors with cognitive impairment, complex medical issues, and co-occurring mental health challenges. Perform cognitive assessments and obtain collateral histories when available. Identify patients who may qualify for ALTCS and coordinate referrals as appropriate. May include seeing patients in both clinic and shelter settings

Position requires extended periods of sitting and standing; Position requires regular bending and reaching, including transfer of patients; Must be able to lift, carry and push 20 pounds on a regular basis; Must be able to lift and carry up to 50 pounds on an occasional basis; Able to adapt to rapid, unpredictable work flow; Able to focus on work at hand with attention to detail; Ability to work with difficult patients and those with mental illness.

Experience and Education:

Graduate of an accredited Physician Assistant or Nurse Practitioner program. Maintain licensure with the State of Arizona to practice as a Physician Assistant or Nurse Practitioner Maintain board certification to practice as a physician assistant or nurse practitioner Certified in Basic Life Support Minimum of three years of experience in either Family Medicine, Internal Medicine or Emergency Medicine Understanding of substance and mental health issues Computer proficiency inclusive of ability to use electronic health records Excellent communication skills Experience in working with underserved populations highly desirable Experience in a community health center preferred #Indeed123
