Identity Hormones and Healthcare is a specialty medical clinic providing affirming, patient-centered care for the LGBTQIA+ community and its allies. With clinics in Phoenix, Denver, Salt Lake City, and Henderson, we are growing our team and seeking a compassionate Per Diem Nurse Practitioner (NP) or Physician Assistant (PA-C) to support our Phoenix, AZ location. Our services include gender-affirming care, hormone replacement therapy (HRT), testosterone replacement therapy (TRT), PrEP/HIV care, STD/STI testing and treatment, weight management, and concierge medicine. This is an excellent opportunity to provide hands-on care while supporting a mission-driven, inclusive clinic. What You'll Do Day-to-Day As a Per Diem NP/PA, you will be a trusted clinician providing high-quality care and guidance for patients seeking long-term health and wellness improvements.
Responsibilities include:
Conducting new patient consultations, reviewing medical histories, and performing physical assessments. Ordering, reviewing, and interpreting lab work and diagnostic tests to guide individualized treatment plans. Prescribing, monitoring, and adjusting hormone therapy (HRT), testosterone replacement therapy (TRT), and related medications. Overseeing weight management treatments, including GLP-1 medications, and monitoring patient progress. Providing clinical oversight for nursing staff performing blood draws, IV therapy, and intramuscular/subcutaneous injections. Reviewing and signing off on EMR charts, labs, and treatment documentation. Ensuring compliance with all state and federal medical regulations, safety standards, and internal protocols. Educating patients on hormone optimization, nutrition, and lifestyle modifications to improve outcomes. Collaborating with clinic leadership and medical directors to ensure consistent, high-quality, patient-centered care.
