Pay Details:

$120,000•$143,000/year Mileage reimbursement Up to 7.5% annual bonus

Schedule:

Monday•Friday, 8:00am•5:00pm

On-Call:

Telephonic Only, once every 6-8 weeks Required Qualifications for a Field Nurse Practitioner /

Physician Assistant:

Active NP License in AZ Certified as a Family Nurse Practitioner or Adult Gerontology Nurse Practitioner or Physician Assistant Minimum 1 year of experience in family practice, general practice, or outpatient care Valid and Active Drivers License Daily Responsibilities for Field Nurse Practitioner /

Physician Assistant:

Conduct comprehensive in-home and community-based visits (minimum 25/week), including assessments, diagnoses, and treatment planning Manage chronic conditions, prescribe/adjust medications, and provide patient education to support long-term health Collaborate with healthcare teams to deliver coordinated, high-quality care and serve as the primary provider of record Benefits for Field Nurse Practitioner /
