  • 100% employer-paid coverage (depending on plan selected) 401(k) with employer match Company-paid life and disability insurance Health Savings Account (HSA) and Flexible Spending Account (FSA) options Generous paid time off and holidays, starting at 168 hours annually, with additional accrual based on tenure A supportive culture focused on teamwork, development, and career advancement You'll make an impact by: • Partner with nephrologists, dialysis teams, and interdisciplinary care providers to deliver comprehensive dialysis care, address patient concerns, and ensure accurate, timely clinical documentation.
  • Manage chronic kidney disease patients across outpatient settings by conducting assessments, developing treatment plans, and coordinating ongoing care in collaboration with physicians.
  • Facilitate seamless transitions of care through annual wellness visits, post-hospital follow-up, medication reconciliation, and patient education to improve outcomes and reduce readmissions.
  • Provide advanced clinical care across multiple settings, including physician offices, skilled nursing facilities, rehabilitation centers, dialysis units, and on-call environments
  • through diagnostic evaluations, treatment planning, and evidence-based interventions.
  • Prescribe medications and therapeutic treatments within scope of practice while delivering compassionate, patient-centered care and maintaining the highest standards of professionalism and clinical excellence.
