Position Summary Modoc Medical Center is seeking Full-Time Physician Assistants and Family Nurse Practitioners for multiple locations within Modoc County. These benefit-eligible positions report to the appropriate Medical Director and Clinic or Facility Manager. The PA or FNP provides comprehensive patient care with a focus on disease prevention, health maintenance, patient education, and collaborative team-based practice. Current opportunities include one role at the Alturas Rural Health Clinic in Alturas, one role at the Canby Family Practice Clinic in Canby, and one role serving both Warnerview Skilled Nursing Facility and Mountain View Skilled Nursing Facility in Alturas. Essential Responsibilities Perform physical examinations, obtain medical histories, and manage acute and chronic conditions Order and interpret diagnostic tests and prescribe medications Provide minor procedures within scope of practice Deliver women's healthcare, prenatal care, family planning, and well-child visits in clinic settings Support admissions, transfers, discharges, and care transitions within the skilled nursing facilities Collaborate with physicians, nurses, therapists, and the broader healthcare team to develop and implement patient or resident care plans Educate patients, residents, and families regarding treatment, medications, and preventive care Maintain accurate and timely documentation Participate in quality improvement efforts and promote safe, evidence-based practice Minimum Qualifications Physician Assistant o Bachelor's degree in health sciences o Graduate of an accredited Physician Assistant program o Current NCCPA certification o Current California PA license and DEA registration if applicable o Current BCLS and ACLS certifications o Ability to complete CME hours and maintain national certification o Strong communication skills, basic computer proficiency, and ability to work with patients and residents of all ages Family Nurse Practitioner o Current California RN license o Master's degree from an accredited Nurse Practitioner program o Current national FNP certification or eligibility o Current BCLS and ACLS certifications o California NP license and DEA registration if applicable o Strong English communication skills, basic computer proficiency, and ability to work with patients and residents of all ages Preferred Qualifications Experience in rural primary care or skilled nursing settings Familiarity with electronic health records and clinical documentation systems Comfort working independently while maintaining strong collaboration with multidisciplinary teams Benefits Relocation Reimbursement (for eligible applicants): Up to 1,500 miles
