We are committed to providing high-quality, personalized care with a focus on preventive medicine and chronic disease management, particularly for adult and Medicare populations. We are building a practice that prioritizes access, continuity, and strong patient relationships—and are looking for a motivated Nurse Practitioner to grow with us. Position Overview We are seeking a compassionate, clinically strong, and reliable Family Nurse Practitioner to join our team. The ideal candidate is comfortable managing a broad range of primary care conditions, values patient relationships, and can work both independently and collaboratively. This role offers the opportunity to be part of a growing practice with increasing patient volume and to play a key role in expanding access to care. Responsibilities Provide comprehensive primary care to adult patients (with option to see pediatrics if desired) Diagnose and manage acute and chronic conditions (e.g., hypertension, diabetes, hyperlipidemia) Perform annual wellness visits, preventive care, and health screenings Manage same-day/sick visits Order and interpret labs, imaging, and diagnostic tests Prescribe medications and develop treatment plans Educate patients on disease management and preventive care Maintain accurate and timely documentation in EMR Collaborate with physician and clinical staff to ensure high-quality care Participate in quality improvement and care coordination efforts Qualifications Current certification as a Family Nurse Practitioner Active California license (or eligibility) Experience in family medicine or primary care preferred Strong clinical judgment and ability to work independently Excellent communication and interpersonal skills Comfortable with EMR systems Preferred (Not Required) Experience with Medicare Advantage patients Spanish-speaking is a plus Experience with value-based care or chronic care management What We Offer Supportive, collaborative work environment Opportunity to grow with a rapidly expanding practice Flexible scheduling options Competitive compensation with potential bonuses Meaningful patient relationships (not a high-volume "assembly line" model) How to Apply Please submit your CV and a brief introduction outlining your experience and interest in the position.
