Professional Duties Utilize proper channels to effectively communicate verbally and in writing. Utilize electronic medical/health record system for documentation and coding. Understands the importance of HCC Coding standards and documents accordingly. Collaborate with the department director and/or VP, Clinical Services Integration to address urgent issues. Understand seniors' needs and endeavor to accommodate as need and when possible. Actively promote End-of-Life decision making. Understand productivity standard expectations. Attend and participate in meetings as requested. Perform other duties as assigned. Good communication skills, detail oriented, critical thinking and problem-solving skills. Excellent customer service and telephone etiquette skills. Valid California driver's license and proof of automobile insurance.
