Job Description

Overview Join our dynamic healthcare team as a Family Nurse Practitioner (FNP), where you will play a vital role in delivering comprehensive, patient-centered care across diverse clinical settings. This energetic position offers the opportunity to work with patients of all ages, from pediatrics to geriatrics, providing preventive, acute, and chronic care. As an FNP, you will utilize your advanced nursing skills to assess, diagnose, and manage a wide range of health conditions while collaborating with multidisciplinary teams to ensure optimal patient outcomes. Responsibilities Conduct thorough patient assessments, including physical examinations and vital signs, to determine health status and formulate treatment plans. Provide primary care services such as health screenings, immunizations, health education, and disease prevention strategies. Manage acute and chronic illnesses by prescribing medications, performing procedures like suturing and IV infusions, and ordering diagnostic tests including sonography and laboratory evaluations. Utilize electronic health record (EHR) systems such as Epic or eClinicalWorks for accurate medical documentation and coding (ICD-10/ICD-9/CPT). Collaborate with specialists in areas like gastroenterology, infectious disease care, behavioral health, geriatrics, and hospice & palliative medicine to coordinate comprehensive patient care. Participate in clinical research initiatives or quality improvement projects to enhance treatment protocols and patient safety standards. Provide telehealth consultations to expand access to care and support patients remotely across various healthcare settings including outpatient clinics and home care environments. Requirements Valid Family Nurse Practitioner certification with current state licensure. Proven experience in medical office settings with familiarity in EMR/EHR systems such as Cerner or Athenahealth. Strong clinical skills including patient assessment, medication administration, suturing, phlebotomy, catheterization, and sterile processing techniques. Experience working with diverse populations including pediatrics, seniors, individuals with disabilities or developmental disabilities, and those in assisted living or nursing home environments. Knowledge of acute care practices such as ICU experience, emergency medicine procedures like airway management or spinal taps, and hospital medicine protocols. Ability to perform complex procedures including dermal fillers or esthetic laser treatments is a plus. Excellent communication skills for triage calls, patient education, discharge planning, and case management activities.
