Job Overview We are seeking an experienced and highly skilled Mobile Wound Care Provider (Nurse Practitioner or Physician) to join our growing wound care team. This position involves providing advanced mobile wound care services to patients in skilled nursing facilities, assisted living facilities, rehabilitation centers, and other long-term care settings throughout the assigned territory. The provider will be responsible for evaluating, diagnosing, treating, and managing patients with chronic and acute wounds including pressure ulcers, diabetic foot ulcers, venous stasis ulcers, arterial ulcers, post-surgical wounds, skin tears, and other non-healing wounds. The ideal candidate must be comfortable working independently in a mobile setting while delivering high-quality, patient-centered care. Candidates must have a minimum of one year of wound care experience and at least one year of hands-on experience performing wound grafting procedures and advanced wound care treatments. Applicants must hold an active wound care certification such as WCC, CWS, CWCN, or equivalent. Experience with skin substitutes, biologic graft applications, debridement, infection management, offloading techniques, and wound prevention protocols is strongly preferred. The provider will perform comprehensive wound assessments, develop individualized treatment plans, monitor wound healing progress, perform bedside procedures, document all patient encounters accurately within the EMR system, and coordinate care with facility staff, primary care providers, specialists, and patient families. Strong knowledge of wound staging, infection prevention, pressure injury management, and Medicare-compliant documentation is required. This role requires excellent clinical judgment, strong bedside manner, attention to detail, and the ability to manage a high patient volume efficiently while maintaining exceptional standards of care. Providers must be dependable, organized, and capable of traveling between multiple facilities throughout the day. Requirements include an active California NP or MD/DO license, active wound care certification, reliable transportation, and the ability to work independently within a fast-paced mobile medical environment.. Responsibilities Conduct comprehensive wound assessments, including evaluating wound size, depth, exudate, and signs of infection. Develop and implement individualized wound care plans utilizing evidence-based practices and advanced modalities such as dermal fillers or laser treatments when appropriate. Perform procedures including suturing, debridement, dressing changes, and catheter care with sterile technique. Manage complex cases involving diabetic foot ulcers, pressure injuries, surgical wounds, and other chronic wounds across various healthcare settings. Collaborate with multidisciplinary teams to coordinate patient care, including working with physical therapists, nutritionists, and case managers. Document all patient encounters accurately within electronic health record (EHR) systems like Epic or eClinicalWorks while adhering to HIPAA regulations. Provide patient education on wound prevention, self-care techniques, and lifestyle modifications to promote healing and prevent recurrence. Requirements Proven experience in wound management within outpatient clinics or hospital settings; experience with acute care or critical care environments is highly valued. Familiarity with advanced wound treatment modalities such as botulinum toxin treatment or esthetic laser therapy is a plus. Proficiency in electronic medical records systems including Epic, Cerner, or Athenahealth; telehealth experience is advantageous. Strong knowledge of medical terminology, anatomy, physiology related to skin and tissue healing processes. Ability to perform clinical procedures such as IV infusion, venipuncture, catheterization, and sterile dressing changes confidently.
