Professional Wound Specialists We are seeking a dedicated and experienced mobile Wound Care Nurse Practitioner or Physician Assistant to join our specialized healthcare team. As a mobile wound care provider, you will travel to various facilities, hospitals and patient homes throughout the West Los Angeles area, delivering comprehensive wound care management directly to patients who need specialized treatment.

This is a mobile position requiring travel to skilled nursing facilities, assisted living communities, and patient homes Reliable personal transportation is required

Responsibilities:
Conduct thorough on-site assessments of patients' wounds, including wound measurements, tissue assessment, and identification of underlying conditions Develop and implement individualized treatment plans tailored to patients' specific wound care needs, including debridement, dressings, and adjunctive therapies Collaborate with facility staff, primary care providers, and interdisciplinary healthcare team members to ensure comprehensive patient care and optimal treatment outcomes Educate patients, families, and facility caregivers on proper wound care techniques, prevention strategies, and lifestyle modifications Monitor patients' progress through regular visits and adjust treatment plans accordingly to promote optimal healing outcomes Maintain accurate and up-to-date documentation of patient assessments, interventions, and progress notes in accordance with facility and regulatory requirements Manage your own schedule and coordinate patient visits efficiently across multiple locations
Requirements:
Current certification as a Nurse Practitioner OR Physician Assistant Master's degree in Nursing from an accredited institution (for NP candidates) OR Master's degree from an accredited PA program Active state licensure as a Nurse Practitioner or Physician Assistant in California Valid driver's license and reliable personal vehicle with current insurance Minimum of 2 years of experience in wound care management, with proficiency in wound assessment, treatment modalities, and evidence-based practice Experience working independently and managing a mobile patient caseload Strong interpersonal and communication skills, with the ability to effectively collaborate with patients, families, facility staff, and healthcare team members Commitment to providing compassionate, patient-centered care and promoting positive patient outcomes Ability to work flexible hours and travel throughout the service area
Preferred Qualifications:
Certification in wound care (CWCA, CWCN, CWS, or equivalent) Experience in mobile healthcare delivery or home health Experience working in skilled nursing facilities, long-term care, or outpatient wound care settings Proficiency in electronic medical record (EMR) documentation systems Bilingual capabilities (Spanish preferred given service area demographics)
