Job Description

Mobile wound care NP/PA Curaderm, Inc Coachella Valley, CA Part-time, Full-time Part-time, Full-time Job Overview We are seeking a dynamic and compassionate Mobile Wound Care Nurse Practitioner (NP) or Physician Assistant (PA) to join our dedicated healthcare team. In this role, you will deliver specialized wound management services directly to patients in various settings, including assisted living facilities, board and cares, and homes. Your expertise will help improve healing outcomes, prevent complications, and enhance patient quality of life through comprehensive wound assessment and treatment. This position offers an exciting opportunity to work in a fast-paced environment where your clinical skills and proactive approach will make a meaningful difference. Responsibilities Conduct thorough wound assessments, including measuring, photographing, and documenting wound progress using electronic health record (EHR) system. Develop individualized treatment plans incorporating advanced wound care techniques like debridement, dressing selection, and advanced wound care. to include grafting. Manage complex cases involving acute pain management, infection control, and comorbidities like diabetes or vascular disease. Collaborate with multidisciplinary teams including physicians, nurses, physical therapists, and case managers to coordinate patient care across inpatient, outpatient, and home care settings. Provide education to patients and caregivers on wound prevention strategies, nutrition, mobility exercises, pressure injury mitigation, and medication administration to promote healing. Utilize telehealth platforms for remote consultations when applicable while maintaining HIPAA compliance and high standards of patient confidentiality. May require supervision of LVNs. Requirements Current licensure as a Nurse Practitioner or Physician Assistant with valid state certification. Proven experience in wound care management within diverse clinical environments such as hospitals (including Level I Trauma Centers), nursing homes, or outpatient clinics. Proficiency with EMR/EHR systems Strong knowledge of physiology, anatomy, infection control protocols, and advanced wound treatment modalities including skin grafts. Experience working with vulnerable populations such as elderly patients in geriatrics or memory care units; familiarity with assisted living facilities preferred. Ability to handle complex cases involving acute pain management, and infectious disease care. Excellent patient assessment skills complemented by critical thinking.
