Job Description

Job Overview We are seeking a dynamic and compassionate Nurse Practitioner to join our healthcare team, dedicated to delivering comprehensive primary care across diverse patient populations. In this vital role, you will provide high-quality medical services, perform thorough patient assessments, and develop personalized treatment plans This position offers an exciting opportunity to make a meaningful impact on community health while working within a collaborative and innovative environment. Responsibilities Conduct detailed physical examinations and patient assessments, including vital signs, medical history reviews, and diagnostic evaluations. Diagnose and manage acute and chronic illnesses across multiple specialties such as internal medicine. Prescribe medications adhering to best practices and regulatory standards. Utilize electronic health record (EHR) system Practice Fusion for accurate documentation of patient encounters and medical records. Collaborate with multidisciplinary teams to coordinate discharge planning, case management, and utilization review for optimal patient outcomes. Provide telehealth consultations Educate patients on health coaching topics including nutrition, family planning, memory care, hospice & palliative medicine, and disease prevention strategies. Experience Proven experience as a Nurse Practitioner with a strong background in primary care Demonstrated proficiency in working with diverse populations Hands-on experience with procedures like venipuncture (phlebotomy), catheter care, airway management (ventilators), spinal taps, gastrostomy tube feeding, and sterile techniques. Familiarity with EMR/EHR systems, Practice Fusion Knowledge of CPT coding (Current Procedural Terminology), ICD-9/10 coding systems for accurate documentation and billing processes. Experience working in various settings including outpatient clinics, inpatient units like ICU or PICU (Pediatric Intensive Care Unit), nursing homes, assisted living facilities, or post-acute care environments. Additional expertise in areas such as occupational health, infectious disease care (including infection control protocols), hospice & palliative medicine, trauma response at Level II or Level I trauma centers is preferred. Pay:
$123,990.20 - $149,321.53 per year
