Job Description

Job Summary We are seeking a dedicated and highly skilled and autonomous healthcare provider specializing in the assessment, diagnosis and management of acute and chronic wounds to join our healthcare team. The ideal candidate will provide high-quality patient care in various settings. This role requires a strong background in patient assessment and wound care treatment . The Nurse Practitioner will collaborate with physicians and other healthcare professionals to ensure comprehensive care delivery. Duties Conduct thorough physical examinations and assessments of patients. Delivers comprehensive wound care services including debridement, infection control, advanced dressing selection and patient education. Independently conducts evaluations, develops individualized treatment plans, and coordinates with multidisciplinary teams when needed. Utilizes evidence-based practices to promote optimal healing, prevent complications, and improve patient quality of life. Maintains accurate medical records, ensures compliance with state and federal regulations, and manages operations of a self-directed wound care practice. Utilize EMR/EHR systems effectively for medical documentation and patient records. Provide health coaching and education to patients regarding their conditions and treatments. Qualifications Current Nurse Practitioner license with relevant certifications. Experience in critical care, emergency medicine, or hospital medicine preferred. Proficiency with EMR systems. Familiarity with
ICD-9/ICD-10
coding and CPT coding practices. Strong knowledge of medical terminology, anatomy, physiology, and infection control protocols. Experience working in various settings including urgent care, outpatient clinics, nursing homes, or home care environments. Excellent communication skills for effective patient interaction and education.
