Job Description

Nurse Practitioner / Physician Assistant (Pediatric, Wellness, Primary Care, Cardiology)
Location:
Coachella Valley, CA (Multiple Sites)
Status:
Full-Time, Permanent We are seeking experienced Nurse Practitioners or Physician Assistants passionate about wellness, preventative medicine, and high-quality outpatient care. This role focuses on senior and commercial patient populations with an emphasis on chronic disease management, preventative care, care coordination, and managed healthcare. Providers will conduct comprehensive wellness visits, manage chronic conditions, interpret diagnostics and laboratory results, provide patient education, and collaborate with multidisciplinary teams to optimize patient outcomes. The role includes direct patient care in clinic, coordination with case managers and other providers, participation in quality and utilization initiatives, and maintaining accurate and timely EMR documentation.
Responsibilities include:
Managing adult and geriatric patients in an outpatient primary care/wellness setting Conducting physical assessments, interpreting labs/EKGs, and developing treatment plans Managing chronic conditions and preventative care needs Collaborating with physicians, NPs/PAs, RNs, pharmacists, and care teams Participating in quality improvement, HCC initiatives, and managed care programs Providing patient and family education, including care planning discussions Maintaining compliance with HIPAA and clinical documentation standards Supporting clinic coverage, including occasional telephonic call rotation
Qualifications:
Active California NP or PA license and DEA certification Experience in primary care, wellness, urgent care, or outpatient managed care preferred Strong understanding of chronic disease management and preventative medicine Experience with EMR systems and managed care environments Ability to work collaboratively within multidisciplinary teams Bilingual Spanish/English preferred for some locations
Schedule :
Outpatient setting Flexible clinic-based schedule, including four 10-hour shifts or standard weekday shifts depending on location Patient-focused pace allowing comprehensive wellness visits
