The ideal candidate will provide comprehensive, culturally competent care to patients with chronic conditions, such as diabetes, hypertension, and heart disease, with a focus on prevention, patient education, and health equity. Key Responsibilities 1.
Patient Care:
Perform comprehensive health assessments, including detailed medical histories and physical examinations for patients with chronic illnesses. Develop, implement, and monitor personalized treatment plans to manage chronic diseases and promote optimal health outcomes. Prescribe and manage medications, treatments, and lifestyle modifications, monitoring effectiveness and adherence. Conduct preventive care and screenings aligned with current clinical guidelines. 2.
Chronic Disease Management:
Collaborate with patients to set realistic self-management goals and offer ongoing education and encouragement. Address social, cultural, and linguistic factors affecting chronic disease management, ensuring care plans are culturally sensitive and accessible. 3.
Bilingual Patient Advocacy & Education:
Provide education to Spanish-speaking patients and families about chronic disease prevention, management, and lifestyle changes in their native language. Act as a cultural bridge to reduce language barriers and improve patient trust, understanding, and adherence to care plans. Advocate for patient needs, addressing social determinants of health that impact outcomes. 4.
Quality Improvement & Collaboration:
Participate in quality improvement initiatives to enhance patient outcomes and satisfaction. Work collaboratively with physicians, care coordinators, and other healthcare professionals to deliver comprehensive, team-based care. 5.
Community Engagement:
Engage in outreach initiatives to promote chronic disease prevention and education within the local Spanish-speaking community. Serve as a trusted resource to address disparities in access to healthcare services.
Qualifications Education:
Master's or Doctoral degree in Nursing from an accredited program. Certified as a Nurse Practitioner (NP) in California with a focus on family practice, primary care, or adult-gerontology.
Licensure & Certification:
Active Advanced Practice Registered Nurse (APRN) license. National certification through AANP or ANCC. Bilingual proficiency in Spanish and English (required).
Experience:
Minimum of 2 years of clinical experience in chronic disease management, primary care, or a similar setting preferred. Experience providing culturally competent care to diverse populations, particularly Spanish-speaking communities.
Skills:
Advanced clinical assessment and diagnostic skills. Exceptional communication and patient education abilities, with fluency in Spanish. Proficiency in EHR systems and data-driven care planning. Strong understanding of health disparities and commitment to health equity. Work Environment Outpatient clinical setting with occasional community-based outreach. Frequent collaboration with interdisciplinary teams to optimize care coordination. Focus on providing care tailored to the linguistic and cultural needs of Spanish-speaking patients.
