Job Description
Job Overview Join our dynamic healthcare team as a Family Nurse Practitioner (FNP), where you will play a vital role in delivering comprehensive, patient-centered care across diverse populations. This energetic position offers the opportunity to work in outpatient clinics, urgent care settings, and specialized clinics, providing primary care, acute illness management, and health promotion. You will utilize your clinical expertise to assess, diagnose, and treat patients of all ages, ensuring high-quality outcomes while fostering trusting relationships. Your proactive approach and passion for holistic health will make a meaningful difference in patients' lives every day. Duties Conduct thorough physical examinations and patient assessments to determine health status and develop individualized care plans. Manage acute and chronic conditions across various specialties including pediatrics, geriatrics, behavioral health, and internal medicine. Administer medications, including injections and IV infusions, and perform procedures such as suturing, catheterization, and dermal fillers. Utilize EMR (Electronic Medical Records) systems like Epic, eClinicalWorks, or Cerner to document patient encounters accurately and efficiently. Coordinate with multidisciplinary teams for discharge planning, case management, and referrals to specialists such as gastroenterology or infectious disease care. Provide telehealth services to expand access to care while maintaining HIPAA compliance and ensuring patient confidentiality. Engage in health coaching, patient education on medication administration, nutrition, and disease prevention strategies. Perform diagnostic evaluations using laboratory experience or medical imaging when necessary to inform treatment decisions. Experience Proven experience as a Nurse Practitioner with a focus on primary care or urgent/emergency medicine settings. Demonstrated proficiency with EMR/EHR systems such as Epic or Athenahealth; telehealth experience is highly desirable. Strong background in managing complex cases involving acute pain management, infectious diseases, geriatrics, or behavioral health. Experience performing procedures including suturing, dermal fillers, spinal taps, or catheterization is preferred. Critical care experience from ICU or Level I/II trauma center environments is advantageous. Knowledge of CPT coding (Current Procedural Terminology), ICD coding (International Classification of Diseases), and utilization review processes. Familiarity with working with diverse populations such as assisted living residents, hospice & palliative medicine patients, or individuals with developmental disabilities enhances your fit for this role.
