Role:
In this position, you will provide comprehensive primary care services to a diverse adult and family patient population. You will play a key role in chronic disease management, preventive care, and patient education, ensuring continuity and quality of care. Working closely with physicians and support staff, you will help drive better health outcomes and an exceptional patient experience.
Position Offers:

Key Responsibilities:
Conduct comprehensive patient assessments, including histories, physical exams, and diagnostic evaluations in a primary care setting Manage and follow patients with chronic conditions such as diabetes, hypertension, COPD, and cardiovascular disease Develop, implement, and adjust evidence-based treatment plans, including medication management and prescribing Provide patient and family education on disease processes, treatment plans, lifestyle modifications, and preventive care Order, interpret, and act on diagnostic tests and lab results, coordinating care as needed with other providers Document all patient encounters, orders, and care plans accurately and promptly in the electronic health record (EHR) Collaborate with physicians, nurses, and ancillary staff to ensure coordinated, high-quality care Participate in care management activities, including follow-up visits, medication reconciliation, and transitions of care
Qualifications:
Master's degree in Nursing from an accredited Nurse Practitioner program required Active California NP license in good standing required Current BLS certification required; ACLS certification preferred Prior experience in an outpatient primary care or community health setting Bilingual skills (e.g., English/Spanish) but not required If you are a dedicated Nurse Practitioner seeking a stable primary care role with a 4-day work week and the opportunity to make a lasting impact, we encourage you to apply.
