$175•275 per visit (based on experience, credentials, and procedures performed)

Flexible | Daytime Hours Job Summary We are seeking an experienced and motivated Nurse Practitioner (NP) to join our growing mobile healthcare team . This role focuses on delivering high-quality, patient-centered care in home-based and facility settings. Ideal candidates are comfortable working independently, performing advanced clinical procedures, and managing complex patients in a mobile environment. Responsibilities Perform comprehensive patient assessments, including medical history, physical examinations, and risk evaluations. Diagnose, treat, and manage chronic conditions within scope of practice. Develop, implement, and revise individualized plans of care. Wound assessments and staging Sharp, mechanical, and/or enzymatic debridement (as credentialed) Application of skin substitutes and allografts Selection and management of advanced wound dressings Ongoing wound monitoring and documentation Perform and interpret diagnostic tests, lab results, and point-of-care testing. Educate patients and caregivers on diagnoses, treatment plans, wound care, medications, and preventive health. Deliver care in patient homes, skilled nursing facilities, and community settings. Coordinate care with physicians, nurses, therapists, case managers, and facility staff. Ensure accurate, timely documentation in the electronic medical record (EMR). Maintain compliance with HIPAA, Medicare/Medicaid, payer guidelines, and regulatory standards. Follow company clinical protocols, safety policies, and quality improvement initiatives.
Pay:
$175.00•$275.00 per hour
