Nurse Practitioner (NP)
Location:
San Bernardino, CA Overview:
Our well-established primary care practice is seeking a Nurse Practitioner to provide comprehensive care to an adult and family patient population. This role focuses on chronic disease management, preventive care, and patient education while working collaboratively within a supportive, team-based environment.
Position Offers:
Monday-Thursday schedule (4-day work week) No nights or weekends Competitive pay: $80-$85/hour DOE Professional development opportunities Temporary or permanent placement options
Key Responsibilities:
Conduct comprehensive patient evaluations, including medical histories, physical examinations, and clinical assessments Manage chronic conditions such as diabetes, hypertension, COPD, and cardiovascular disease Develop, implement, and adjust individualized treatment and medication plans Provide patient and family education on preventive care, treatment plans, and healthy lifestyle practices Order, interpret, and coordinate follow-up care based on laboratory and diagnostic test results Maintain accurate and timely documentation within the electronic health record Collaborate with physicians and interdisciplinary team members to ensure coordinated patient care Support care management activities, including patient follow-ups and transitions of care
Qualifications:
Master's degree in Nursing from an accredited Nurse Practitioner program Active California Nurse Practitioner license in good standing Current BLS certification; ACLS preferred Previous experience in outpatient primary care or community health preferred Bilingual English/Spanish skills are a plus
