Nurse Practitioner/Physician Assistant CHP Health San Diego, CA Job Details $95 - $100 an hour 9 hours ago Qualifications Collaborate with healthcare professionals DEA License Advanced practice provider Multidisciplinary team collaboration for treatment planning Full Job Description Advanced Practice Provider (NP/PA) Correctional Healthcare Partners (CHP) Overview Correctional Healthcare Partners (CHP) is seeking a dedicated and compassionate Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to deliver high-quality, patient-centered care within a correctional setting. This role offers the opportunity to practice at the top of your license, providing comprehensive medical care to an underserved population while working collaboratively within a multidisciplinary team. The APP will play a key role in improving patient outcomes, ensuring continuity of care, and supporting CHP's mission of delivering accessible, quality healthcare. Key Responsibilities Perform comprehensive health assessments, including physical exams, medical histories, and diagnostic evaluations Diagnose and manage acute and chronic medical conditions Develop and implement individualized treatment plans Prescribe medications and treatments in accordance with state regulations and CHP protocols Order, interpret, and evaluate diagnostic tests and lab results Provide patient education on disease prevention, health maintenance, and treatment plans Respond to medical emergencies and provide urgent care as needed Collaborate with physicians, nursing staff, mental health clinicians, and custody staff to ensure coordinated care Maintain accurate, timely, and compliant documentation in the electronic health record (EHR) Participate in quality improvement initiatives and adherence to clinical protocols Support discharge planning and continuity of care upon release or transfer Qualifications For Nurse Practitioners (NP): Active and unrestricted NP license in the applicable state Board certification (FNP, AGNP, or related) Current DEA license (or ability to obtain prior to start) For Physician Assistants (PA): Active and unrestricted PA license in the applicable state Graduate of an accredited PA program NCCPA certification required Current DEA license (or ability to obtain prior to start) Minimum 2-3 years of APP experience in primary care, urgent care, emergency medicine, or a related setting Experience in correctional healthcare or working with underserved populations preferred but not required Strong clinical assessment and decision-making skills Ability to work independently within scope while collaborating with supervising physicians and interdisciplinary teams Excellent communication and interpersonal skills Work Environment Practice in a structured, secure correctional facility Team-based, collaborative care model with physician support High level of autonomy with diverse clinical exposure Fast-paced environment treating a wide range of conditions

Schedule:
12pm to 10pm on Thursdays, Fridays, Saturdays, and Sundays
