This clinic has served the San Jose community for decades, providing high-quality, culturally and linguistically accessible primary care to one of Silicon Valley's most diverse patient populations. It is a stable, mission-driven organization with experienced clinical leadership, physician collaboration built into the care model, and a team that is genuinely supportive of providers. Monday-Friday schedule. No weekends. No evenings. Rotational after-hours call, phone-based. Schedule & Structure Monday-Friday | Outpatient only No weekend or evening clinical hours After-hours call on a rotational basis — phone coverage only Full-time, 40 hours/week Compensation & Benefits $158,000 - $185,000 annually, depending on experience Comprehensive medical, dental, and vision — family coverage available Retirement plan Malpractice/liability coverage provided Annual CME time and financial support Paid licensure and DEA renewal Clinical Environment This FQHC operates with a fully integrated, team-based model. You will work collaboratively with supervising physicians, Medical Assistants, Care Coordinators, Managers, and behavioral health clinicians embedded in the clinic. Standing orders and structured workflows are in place to support efficient, high-quality care — so you can practice at the top of your NP license from day one.
On-site resources include:
Integrated behavioral health — co-located, collaborative, not referral-dependent Electronic Health Records with structured workflows Community health workers and patient educators Chronic disease management programs and group visit infrastructure Quality improvement infrastructure with outcomes tracking and clinical dashboards About the Role You will deliver comprehensive primary care to a diverse patient population across all ages — preventive care, chronic disease management, acute visits, and behavioral health integration. The patient community is predominantly Latino, Vietnamese, and Asian Pacific Islander, with many patients whose primary language is Mandarin, Cantonese, Spanish, or Vietnamese. Bilingual proficiency is preferred but not required — we welcome providers committed to culturally responsive care.
Key responsibilities include:
Full-scope Family Medicine NP practice: preventive, acute, and chronic disease management Independent and collaborative patient management under physician supervision per California NP practice standards Patient and family health education, including group office visits and disease self-management support Collaboration with integrated behavioral health colleagues on shared patients Referrals and care coordination with specialists and community resources as needed Participation in quality improvement, peer review, and outcomes reporting Rotational on-call phone coverage
Qualifications Required:
Master's or Doctoral degree from an accredited FNP program Current California RN license, unrestricted 2 years of experience as an FNP California Nurse Practitioner Furnishing License (or in process) National board certification as a Family Nurse Practitioner (AANP or ANCC) — or board eligible with certification completed within 6 months of hire Active or in-process DEA license Current BLS certification
Preferred:
Bilingual in Mandarin, Cantonese, Spanish, or Vietnamese Prior experience or interest in community health, FQHC, or underserved populations Experience with Epic or similar EHR platforms
Pay:
$158,000.00 - $185,000.00 per year
