With a highly selective patient panel of approximately 200 individuals and a waitlist for new membership, we deliver ultra-personalized, high-touch healthcare that blends the best of Western medicine with advanced integrative and preventative strategies. Our approach is rooted in genuine connection, unhurried time, and truly customized care. We offer house calls, same-day visits, 24/7 access for members, and a refined aesthetic that reflects the elegance and tranquility of the Montecito lifestyle. Role Description We are seeking an exceptional Concierge Nurse Practitioner (NP) or Physician Assistant (PA) to join our practice as a key clinical team member. We're open to the right individual at either part time or full time on-site in Montecito, CA. The ideal candidate will augment and elevate our personalized care model while supporting continued brand growth. In this role, you will work closely with the lead physician to deliver seamless, high-touch care collaborative care to discerning patients who expect excellence in both clinical outcomes and service. You will help extend our integrative philosophy through proactive wellness, follow-up care, and optimization protocols in an intimate, refined environment. We'll work as a team to divide and conquer acute care and chronic care needs. Proficiency and comfort in primary care a must. Advanced understanding of integrative medicine or longevity medicine a plus. Key Responsibilities Provide comprehensive primary and preventative care with a strong emphasis on personalization and clinical excellence. Conduct or contribute to wellness visits, executive physicals, advanced screening follow-ups, and personalized health planning. Manage chronic conditions with a root-cause, integrative approach while focusing on vitality, longevity, and prevention. Support integrative therapies including peptide protocols, nutraceutical optimization, IV nutrient therapies, and lifestyle interventions. Coordinate care with specialists and assist with interpretation of advanced diagnostics (cardiology, cancer screening, preventative imaging, gut health, metabolic panels, etc.). Perform house calls and same-day visits as needed to maintain our signature high-touch service. Build meaningful, long-term relationships with high-achieving patients who value presence, warmth, and clinical artistry. Contribute to the ongoing refinement of clinical protocols and the overall patient experience as we thoughtfully grow the practice. The ideal candidate is a clinically outstanding and creatively curious Nurse Practitioner who thrives in a collaborative, low-volume, high-end setting where every detail matters and medicine is practiced as both science and art. Qualifications Nurse Practitioner degree (NP) or Physician Assistant (PA) degree from an accredited program Active California medical license and DEA registration National Board Certification Minimum 3+ years of clinical experience, with strong preference for concierge, integrative, functional, executive health, or premium primary care environments Genuine experience and interest in advanced preventative and integrative strategies, including: Cardiovascular risk assessment and advanced lipidology Cancer screening and early detection Gut microbiome optimization Nutritional supplementation, peptides, and longevity medicine Exceptional interpersonal skills with a warm, polished, and professional presence Strong emotional intelligence and natural ability to connect with discerning, high-profile patients Highly organized with excellent EHR proficiency Creative thinker who enjoys collaborating on innovative, personalized care plans Alignment with our values of discretion, excellence, and patient-centered luxury Preferred Previous experience in concierge, membership-based, or integrative medicine is highly desirable.
