In this vital role, you will utilize your advanced nursing skills to assess, diagnose, and manage patient health needs, ensuring comprehensive care that promotes wellness and recovery. Your expertise will support a wide range of medical services, from acute care to chronic disease management, all while fostering positive patient experiences. Duties Conduct thorough patient assessments for especially for Medicare patients, including vital signs, medical histories, and physical examinations for geriatrics, and adults Develop and implement individualized treatment plans in collaboration with physicians and other healthcare professionals Administer medications and injections, including botulinum toxin treatments for aesthetic or therapeutic purposes Perform specialized procedures such as suturing, and dermal filler injections Manage urgent and acute cases by triaging patients effectively in outpatient Utilize electronic health record (EHR) systems for accurate documentation of patient encounters and medical records Engage in telehealth consultations to extend care access remotely while maintaining HIPAA compliance and patient confidentiality Coordinate with multidisciplinary teams for discharge planning, case management, and post-acute care services Support diagnostic evaluations using sonography when necessary Provide education on health coaching, medication adherence, nutrition, and disease prevention strategies Requirements Valid Nurse Practitioner license with current certification to practice independently in CA, for Medicare patients. Proven experience in hospital rounding is highly desirable Strong background in internal medicine, geriatrics is desirable. Hands-on experience with EMR/EHR systems for efficient medical documentation Proficiency in performing procedures such as suturing, arthrocentesis and dermal fillers Knowledge of CPT coding (Current Procedural Terminology), ICD-10/ICD-9 coding standards for billing and documentation purposes Familiarity with infection control protocols and aseptic techniques to ensure patient safety during procedures Excellent patient assessment skills with the ability to handle complex cases involving acute pain management or infectious diseases Ability to work effectively within fast-paced environments such as hospitals , outpatient clinics, or assisted living facilities Strong communication skills for collaborating with healthcare teams and educating patients across diverse populations including seniors Commitment to maintaining HIPAA compliance and safeguarding patient confidentiality at all times Join us in delivering energetic healthcare solutions that truly make a difference!
