Job Description
Diabetes Program Nurse Practitioner (Grant Funded) Position Summary This is a grant funded position. The Diabetes Program Nurse Practitioner is primarily responsible for: Providing medical management, care, and education for persons diagnosed with diabetes and related chronic conditions, as well as prevention and wellness Assisting with ongoing program planning, development, and implementation of diabetes-related chronic disease prevention and treatment projects Conducting regular trips to village clinics to provide patient care Collaborating with YKDRH and Sub-regional clinic providers to ensure patients are receiving care according to American Diabetes Association's Standards of Care Providing technical assistance and training to medical staff on diabetes issues and current standards of care Assisting with diabetes-related health promotion projects Visiting tribal councils and functioning as a Diabetes Team Member Believing in and promoting Napartet and all corporate endeavors Position Qualifications Education Master's Degree in the Science of Nursing Preferred Master's Degree in Public Health (MPH) Experience Two (2) years experience in family practice, chronic disease management, or related clinical setting Preferred experience working in public health programming Experience providing patient care and/or teaching in a cross-cultural setting Experience working with database and case management Experience collaborating with a variety of disciplines for project completion Licensure / Certification Nurse Practitioner, national certification, licensed and in good standing or eligible for licensure in the State of Alaska NP must be eligible for a DEA number and eligible for credentialing at YK Delta Regional Hospital in Bethel Preferred Certified Diabetes Educator (CDE) or Board Certified in Advanced Diabetes Management (BC-ADM) Skills / Knowledge Strong knowledge base in the field of diabetes, cardiovascular disease, and public health Computer skills and organizational skills Preferred bi-lingual (Yup'ik / English) Work Style / Abilities Ability to work in a group setting Ability to work in remote villages Ability to work with people of different cultures/backgrounds Ability to be flexible and handle stress Work Environment & Travel Requirements Work in typical office, hospital, and field clinic settings Travel to villages via small plane/sub-arctic conditions Moderate lifting of equipment Occasional long working hours and overnight stays in village clinics up to a few days are required Benefits Generous PTO - starting at 4.5 weeks per year , accrued over time Eleven paid holidays Comprehensive healthcare coverage Life and Disability Insurance Flexible Spending Account Retirement plans Employee Wellness Center Plus more About Our System The Yukon-Kuskokwim Health Corporation, commonly referred to as YKHC, is a Tribal Organization that administers a comprehensive healthcare delivery system for 58 rural communities in southwest Alaska. Each of the communities in our service area is home to a federally-recognized Tribe, whose governing council has authorized YKHC to provide health services on behalf of the federal Indian Health Service (IHS). YKHC's healthcare delivery system includes community clinics, sub-regional clinics in five of the area's larger communities, a regional hospital, dental and optometry services, behavioral health services, counseling and treatment for individuals with substance use disorders, health promotion and disease prevention programs, and environmental health services. Our Community The city of Bethel with approximately 6,000 inhabitants sits on the banks of the Kuskokwim river. As the hub of the Yukon-Kuskokwim Delta, Bethel serves as the center for transportation and culture. The 58 surrounding villages represent a predominantly Yupik Eskimo culture that still relies heavily on their hunter-gatherer traditions.
