Job Description

Job Overview Join our dynamic healthcare team as an Adult / Family Nurse Practitioner, where you will play a vital role in delivering comprehensive, patient-centered care across Long term Care/ Rehabilitation setting. This energetic position offers the opportunity to work with a geriatrics population, and individuals with complex medical needs. You will utilize your clinical expertise to perform assessments, develop treatment plans, and coordinate care to improve health outcomes. Waterbury New Britain Mystic Chester Waterford Stratford Newington Windam West Hartford Duties Conduct thorough physical examinations and patient assessments across nursing homes, and assisted living facilities. Manage acute and chronic conditions by developing personalized treatment plans utilizing evidence-based practices. Administer injections, including immunizations, IV therapy and perform suturing. Perform diagnostic evaluations such as sonography, catheterizations, and specimen collection/processing to support accurate diagnoses. Utilize EMR (Electronic Medical Records) systems like Point Click Care and Gherimed for detailed medical documentation and coding (ICD-10/ICD-9/CPT). Provide triage services via telehealth or in-person to prioritize patient needs effectively. Collaborate with multidisciplinary teams on discharge planning, case management, and utilization review to ensure seamless patient transitions across care settings. Experience Proven experience working as an Adult or Family Nurse Practitioner in diverse clinical environments such as hospitals, emergency rooms, ICU (Intensive Care Unit), or Level I/II trauma centers. Demonstrated proficiency with EMR/EHR systems. Strong background in managing complex cases involving acute pain management, infectious disease care, hospice & palliative medicine, or behavioral health. Experience performing procedures such as IV insertions/infusions, venipuncture, catheter care, and sterile processing techniques. Knowledge of medical coding (ICD-10/ICD-9), CPT coding for billing purposes. Prior work in settings involving geriatrics, memory/dementia care or working with individuals with developmental disabilities is preferred.
