Setting:
Outpatient Retail Health Clinic Position Overview We are seeking an experienced Nurse Practitioner to join a collaborative care team in Norwich, Connecticut. This patient-facing role focuses on delivering evidence-based primary and family care while working alongside a dedicated clinical support team to improve patient outcomes. The ideal candidate will have recent experience in primary care, family medicine, or urgent care and be comfortable practicing autonomously while providing high-quality patient care.
Important:
Candidates must have prior Primary Care or Urgent Care Nurse Practitioner experience. Candidates without this experience will not be considered. Responsibilities Provide comprehensive primary and family care services Assess, diagnose, treat, and manage acute and chronic health conditions Perform routine physical examinations and health assessments Provide preventive care, wellness visits, and health screenings Educate patients on healthy lifestyle choices and disease management Develop and coordinate comprehensive treatment plans Collaborate with physicians, clinical staff, and care team members Conduct patient follow-up and care coordination Verify insurance coverage and collect patient payments Maintain clinic inventory and supplies Open and close the clinic as needed Maintain a clean, safe, and compliant clinical environment Required Qualifications Master's Degree from an accredited Family Nurse Practitioner program Current National Board Certification Active Nurse Practitioner license in the state of Connecticut Minimum 1 year of recent Nurse Practitioner experience in: Primary Care Family Medicine Urgent Care Current BLS Certification through AHA or ARC Strong clinical assessment and diagnostic skills Experience managing chronic disease conditions Excellent communication and patient education skills Proficiency with Electronic Medical Records (EMR/EHR) systems Ability to practice independently and manage multiple priorities Preferred Skills Strong organizational and time management skills Experience working in collaborative care models Comfort with technology and information management systems Ability to adapt to changing patient volumes and clinic needs Willingness to obtain additional state licenses if needed Schedule & TrainingRegular Schedule 3 days per week 8:30 AM - 7:30 PM Additional hours may be available Holiday rotation may be required No requirement to work every other weekend Training Schedule Four-week onboarding period 4 ten-hour shifts or 5 eight-hour shifts weekly Training schedule is flexible Candidate Questionnaire (Required With Submission) Please include answers to the following questions on the resume or submission packet: Do you hold an active NP license in good standing for the state where you are applying?
