Primary Care Physician Assistant or Nurse Practitioner

An established primary care practice in Norwich, CT is seeking a Physician Assistant or Nurse Practitioner to join their team. This will be a full-time role, 4 days per week (32 hours) with a Monday-Friday schedule. No weekends, nights or holidays.

Responsibilities:

Provide outpatient care to patients of all ages Conduct comprehensive patient assessments, including medical history review, physical examinations, and diagnostic evaluations Formulate and implement individualized treatment plans in collaboration with supervising physicians, including medication management, patient education, and coordination of follow-up care Interpret diagnostic tests and imaging studies Provide counseling and support to patients and their families regarding conditions, treatment options, and lifestyle modifications. Collaborate with other healthcare providers, including nurses, medical assistants, and administrative staff, to ensure seamless patient care coordination and workflow efficiency.

Qualifications:

Current licensure as a Nurse Practitioner or Physician Assistant in Connecticut Preferably 1 year of primary care experience Strong clinical assessment and diagnostic skills Excellent communication and interpersonal skills Proficiency in electronic health record (EHR) systems and medical documentation Commitment to maintaining professional competence through ongoing education and participation in professional organizations
