Expertise Training:

Key Responsibilities Conduct thorough assessments of patients, identifying and managing medical conditions under the supervision of a Physiatrist. Develop and implement individualized care plans, focusing on function and rehabilitation to improve patients' quality of life. Collaborate with the healthcare team to ensure comprehensive care, including medication management and therapy recommendations. Educate patients and their families on treatment plans, health maintenance, and preventive care. Monitor and evaluate patient progress, adjusting care plans as necessary to achieve optimal outcomes. Qualifications Licensed Nurse Practitioner with a valid Connecticut certification. Experience working independently while effectively managing your own caseload. Submit accurate and thorough clinical documentation promptly, ensuring compliance with company policies and regulatory requirements.
