APPs will play a critical role in delivering high-access, relationship-driven care through a blend of in-person, virtual, and asynchronous care modalities. APPs will work closely with physicians, behavioral health clinicians, health coaches, and care navigators within a POD-based care structure designed to maximize both patient experience and clinical outcomes. This role is ideal for a clinician who thrives in fast-paced outpatient environments, values strong patient relationships, and is excited about helping build a modern healthcare delivery model from the ground up. Core Responsibilities Provide comprehensive primary care services within scope of practice for adult and pediatric patients Manage same-day, urgent, and low-acuity care needs through in-person, virtual, and asynchronous workflows Support preventive care, chronic disease management, and proactive outreach initiatives Collaborate closely with supervising physicians and interdisciplinary care team members Participate in patient triage, care coordination, medication management, and follow-up care Assist with office-based procedures and musculoskeletal evaluations within training and scope Contribute to after-hours and access support coverage as part of a collaborative team model Leverage digital communication tools and structured intake pathways to improve care efficiency and patient satisfaction Support proactive population health initiatives and predictive outreach efforts Help shape workflows, clinical operations, and patient experience standards within an innovative startup environment Clinical Model The practice utilizes a high-touch, team-based POD model designed around smaller patient panels, rapid access, and integrated care delivery. Each POD supports approximately 500-750 patients and includes physicians, APPs, health coaches/medical assistants, and embedded behavioral health support.
