Job Description

Position Summary We are seeking a dedicated, compassionate Nurse Practitioner (NP) to join our growing clinical team. The NP will serve as a primary provider for adult patients, delivering evidence-based care in a fast-paced yet patient-centered environment. This role offers clinical autonomy, a supportive team structure, and a genuine opportunity to impact community health outcomes. Key Responsibilities Conduct comprehensive health assessments, including history-taking and physical examinations Diagnose and manage acute, episodic, and chronic conditions across the care continuum Order, interpret, and act on diagnostic studies including labs, imaging, and EKGs Develop and implement individualized patient care plans in alignment with clinical guidelines Prescribe medications and therapeutic treatments within scope of practice Deliver preventive care services including screenings, immunizations, and health counseling Coordinate referrals and follow-up care with specialists and community health partners Document all encounters accurately and in a timely manner using the electronic health record (EHR) Educate patients and families on diagnoses, treatment plans, medications, and health promotion Collaborate with physicians, MAs, and administrative staff to ensure seamless patient flow and care delivery Participate in quality improvement initiatives, care coordination rounds, and staff meetings

$75.00 - $80.00 per hour
