Part-time Job Summary:

We are seeking a dedicated and compassionate Primary Care Nurse Practitioner (NP) to join our healthcare team. The NP will provide comprehensive care to patients across the lifespan, focusing on health promotion, disease prevention, and chronic disease management. The ideal candidate will have strong clinical skills, excellent communication, and a commitment to delivering patient-centered care.

Key Responsibilities:

You need to have to have experience in Primary Care, Immigration Physicals and VA-Loyal Veteran Physicals,

• Conduct thorough patient assessments, including medical histories and physical exams and post hospital follow-ups.
• Proficient in HEDIS measurements and scores
• Able to interpret EGK's
• Diagnose and manage acute and chronic conditions such as hypertension, diabetes, and respiratory illnesses.
• Develop and implement individualized treatment plans, including prescribing medications and therapies.
• Order and interpret diagnostic tests such as lab work and imaging studies.
• Provide education on health maintenance, disease prevention, and wellness strategies.
• Collaborate with physicians, specialists, and other healthcare professionals to ensure comprehensive patient care.
• Maintain accurate and timely documentation in electronic health records (EHR).
• Participate in quality improvement initiatives and adhere to best practices in primary care.
• Ensure compliance with state and federal healthcare regulations. Qualification's
• Master's or Doctorate degree in Nursing (MSN/DNP) from an accredited program.
• Current licensure as a Nurse Practitioner (NP) in State of Florida
• Certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Primary Care
• Nurse Practitioner (AGNP) by AANP or ANCC.
• DEA certification and prescriptive authority.
• Minimum of 2 to 3 years of experience in a primary care or outpatient setting preferred.
• Proficiency with eClinical Works EHR systems and familiarity with telehealth services
