The Opportunity Streamline Urology exists to close that gap — by placing credentialed nurse practitioners inside the facility, with clinical pathways and a team behind them. Join our dynamic healthcare team as a Nurse Practitioner specializing in Urology, where you'll deliver expert urological care directly to patients in various post-acute settings. What You Will Do Round on-site at skilled nursing and post-acute facilities on a defined weekly schedule within your assigned territory Assess, diagnose, and manage urological conditions including acute urinary retention, catheter complications, UTI differentiation, neurogenic bladder, and chronic urological disease. Work as an embedded clinical partner to facility nursing teams. Responsibilities Conduct thorough patient assessments, including medical history reviews, physical examinations, and vital signs monitoring to evaluate urological health. Perform diagnostic procedures such as ultrasound sonography and catheterizations, ensuring accurate data collection for effective treatment planning. Manage acute and chronic conditions related to urology, utilizing skills in pain management, infection control, and catheter care. Collaborate with physicians and specialists on complex cases involving trauma, infectious disease care, or post-operative recovery within hospital or outpatient settings. Document all patient interactions meticulously using EMR (Electronic Medical Records) systems like Point Click Care, adhering to HIPAA regulations. Provide patient education on medication administration, lifestyle modifications, and follow-up care plans to promote long-term health management. What Makes This Different Your caseload is yours. Demonstrated experience in urology or related specialties such as nephrology or internal medicine; experience within the post-acute setting is a plus. Proficiency in EMR/EHR systems including Point Click Care, for accurate documentation. Knowledge of CPT coding and
ICD-10/ICD-9
coding standards for accurate medical billing and documentation. Excellent patient care skills with the ability to assess physiology knowledge and perform physical examinations confidently. Ability to work independently in post-acute environments while maintaining high standards of care collaboration and communication. Experience working with diverse populations including geriatric (geriatrics), senior care, assisted living residents, or individuals with developmental disabilities enhances your suitability for this role.
