Job Description

Job Overview We are seeking a dedicated and skilled Nurse Practitioner to join our healthcare team. The ideal candidate will have a passion for providing high-quality patient care, particularly in senior care settings. This role involves diagnosing and treating patients, developing care plans, and collaborating with other healthcare professionals to ensure optimal patient outcomes. The Nurse Practitioner will play a crucial role in enhancing the quality of life for patients, especially those with complex health needs. Responsibilities Conduct comprehensive assessments and diagnostic evaluations of patients. Develop and implement individualized treatment plans based on patient needs. Collaborate with interdisciplinary teams to coordinate patient care effectively. Document patient information accurately using electronic health record systems such as ECW. Engage in clinical research to improve patient care practices and outcomes. Support patients with disabilities by adapting care approaches to meet their unique needs. Educate patients and families on health management, preventive care, and lifestyle modifications. Requirements Current Nurse Practitioner certification and state licensure. Experience in Cardiac care is preferred. Proficiency in diagnostic evaluation and clinical research methodologies. Strong skills in medical nursing practices. Excellent communication, interpersonal, and organizational skills. Ability to work collaboratively within a healthcare team environment.
