Job Description

Job Summary:
Under the direction of the Medical Director and Co-Medical Director, the Nurse Practitioner (NP) ensures the healthcare of incarcerated patients undergoing dialysis treatment within a correctional facility. The NP functions within established professional boundaries to deliver high-quality, evidence-based care from assessment through evaluation, under the continuous supervision of a qualified physician. This includes both in-facility dialysis care and collaboration with multidisciplinary teams across various treatment settings, including outpatient, home dialysis, and skilled nursing/rehabilitation environments. Essential Duties and Responsibilities Manage a clinical caseload of ESRD and pre-ESRD patients within correctional, home dialysis, and nursing home/rehab settings. Perform accurate comprehensive history-taking, diagnosis, subjective findings, physical assessments, and treatment planning. Ensure timely follow up in Electronic Health Records Systems to comply with billing, clinical, and department of corrections goals Deliver high standards of care using advanced autonomous clinical judgment and a broad theoretical knowledge base. Prescribe safe and appropriate medications in accordance with the Florida Board of Nursing and applicable laws. Exhibit excellent communication and written skills, must be detail oriented and be able to multitask effectively. Monitor dialysis patients in partnership with nephrologists and facility staff, ensuring appropriate interventions are implemented. Educate patients on disease processes, dialysis management, lifestyle modifications, and wellness promotion. Collaborate with other healthcare providers, including correctional medical staff and social workers, to ensure continuity of care. Refer patients to external services or specialists when medically indicated. Contribute to the development of new services and patient education programs, such as a Pre-ESRD Education Program. Participate in quality improvement initiatives and help maintain compliance with regulatory and correctional standards. Lead and support the dialysis nursing team to ensure the best practices and high standards of care Demonstrate clear knowledge of Physicians' Dialysis procedures and protocols Must be sensitive to the diverse patient populations Ability to work independently and as part of a multi-disciplinary team
Education/Experience/Qualification:
Master's degree in nursing from an accredited school. Current RN License, active Nurse Practitioner certificate, and furnishing number. Experience in dialysis or nephrology is strongly preferred; correctional healthcare experience is a plus. Current CPR certification, ACLS preferred Current DEA license or eligibility Excellent clinical judgment, communication, and interpersonal skills. Established leadership skills
