In this vital role, you will provide comprehensive mental health assessments, develop personalized treatment plans, and deliver evidence-based psychiatric care across skilled nursing and long term care settings. Responsibilities Conduct thorough psychiatric evaluations, including patient history, mental status examinations, and risk assessments using advanced EMR (Electronic Medical Record). Develop and implement individualized treatment plans incorporating psychopharmacology, psychotherapy, and other modalities aligned with best practices in behavioral health. Prescribe medications responsibly and manage medication regimens while adhering to HIPAA regulations and state-specific scope of practice laws. Collaborate with multidisciplinary teams including primary care providers, social workers, case managers, and specialists in fields such as geriatrics, substance abuse, and trauma care to optimize patient outcomes. Utilize telehealth platforms to extend psychiatric services remotely, ensuring access for underserved populations. Perform patient education on mental health conditions, medication management, lifestyle modifications, and coping strategies to promote long-term wellness. Document all clinical encounters accurately using EMR/EHR systems while maintaining compliance with HIPAA standards. Participate in clinical research projects or quality improvement initiatives aimed at advancing behavioral health practices. Engage in discharge planning, case management activities, and coordination of post-acute care for patients transitioning between inpatient and outpatient environments. Qualifications Valid Florida licensure as a Nurse Practitioner with certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP). Proven experience in behavioral health settings with exposure to diverse populations including seniors requiring memory or dementia care, hospice & palliative medicine patients, or individuals with developmental disabilities. Strong knowledge of psychopharmacology principles and medication administration protocols. Familiarity with EMR/EHR systems for documentation and patient management. Ability to perform examinations and diagnostic evaluations. Skills in medical documentation using CPT coding (Current Procedural Terminology), ICD coding (International Classification of Diseases), ICD-9/10 classifications for accurate billing and reporting purposes. Knowledge of Workers' Compensation law can be beneficial for managing related cases effectively. Excellent communication skills coupled with a proactive approach to patient care coordination across outpatient clinics or inpatient units.
