Job Description

Overview Join our dynamic healthcare team as a Family Nurse Practitioner (FNP), where you will play a vital role in delivering comprehensive, patient-centered care across diverse populations. This energetic position offers the opportunity to work in outpatient clinics, hospitals, and community settings, providing primary care, acute and chronic disease management, and health education. As an FNP, you will utilize your clinical expertise to assess, diagnose, and treat patients of all ages, fostering health and wellness through proactive interventions and compassionate support. Your contributions will directly impact patient outcomes and community health initiatives. Responsibilities Conduct thorough physical examinations and patient assessments to determine health status and develop personalized care plans. Manage a broad spectrum of medical conditions including acute pain, infectious diseases, geriatrics, pediatrics, behavioral health issues, and chronic illnesses. Administer medications, including injections and IV infusions, perform suturing, and deliver Botulinum toxin treatments for aesthetic or therapeutic purposes. Utilize electronic health record (EHR) systems such as Epic or eClinicalWorks to document patient encounters accurately and efficiently. Provide health coaching, education on disease prevention, family planning guidance, and discharge planning for hospitalized patients. Collaborate with multidisciplinary teams in settings like critical care units (ICU), emergency rooms, hospice & palliative care programs, and assisted living facilities. Engage in telehealth consultations to expand access to care while maintaining high-quality standards aligned with HIPAA regulations. Participate in clinical research initiatives or quality improvement projects to advance evidence-based practices within the organization. Requirements Valid Family Nurse Practitioner license with current certification from an accredited program. Proven experience working in diverse clinical environments such as hospital medicine, emergency medicine, ICU or trauma centers (Level I or II trauma experience preferred). Proficiency with EMR/EHR systems including Epic, Cerner or Athenahealth; familiarity with CPT coding, ICD-10/ICD-9 coding is a plus. Strong knowledge of physiology, anatomy, pharmacology (including psychopharmacology), and diagnostic evaluation techniques such as sonography or laboratory specimen processing. Experience managing complex cases involving acute care procedures like spinal taps or catheterizations; familiarity with infection control protocols is essential. Ability to perform procedures such as venipuncture, sterile processing techniques, tube feeding management, and basic life support (BLS). Excellent patient communication skills across age groups—including pediatrics (including toddler and neonatal care), geriatrics with dementia or Alzheimer's disease—and individuals with disabilities or developmental challenges. Demonstrated ability to work collaboratively within multidisciplinary teams in outpatient clinics or inpatient settings while adhering to NCQA standards and Workers' Compensation law requirements.
