Works in collaboration with a physician and other palliative care team members in an active practice to deliver palliative care and related health care services to patients and families in patient homes and a variety of settings.

Schedule:
Days, M-F Responsibilities:
Performs medical examinations and evaluations, diagnoses, treatment, follow-up, consultation, and education for both patients and families. Assists with diagnosis, treatment, and management of acute and chronic health conditions. Orders, performs, and interprets laboratory and radiology tests within the scope of professional practice. Writes orders pertaining to informed consent, diagnostic procedures, and treatments. Prescribes medications and other treatments. Administers medications as prescribed. Consults with palliative care physician, or designee, as needed, informs attending physician of services provided, and collaborates with other physicians as needed. Facilitates clarification of patient and family goals of care. Prepares and maintains accurate patient records, charts, and documents to support sound medical practice and reimbursement of services provided. Executes physician's orders in accordance with legal, ethical, and organizational standards. Complies with applicable laws and regulations. Initiates reimbursement for services rendered on a timely basis. Ensures provision of informed consent for CPR and DNR. Recommends medications as required to treat specific populations. Adheres to the practice of confidentiality regarding patients, families, staff, and the organization. Provides educational sessions to advance palliative medicine among internal staff, the medical community, and the community as a whole. Participates in on-call coverage and assists in establishing schedules. Participates in regular interdisciplinary team meetings to review active cases.
Qualifications:
Graduate, advanced education with a Master's degree Minimum of two (2) years of nursing experience. Experience in hospice palliative care preferred Valid State of Florida Driver's license with driving record acceptable to Community Hospice insurance carrier. Advanced Practice Nurse in the
