Nurse Practitioner - Internist Office

Job Summary We are seeking a dynamic and compassionate Nurse Practitioner specializing in internal medicine to join our busy outpatient clinic. This role offers an exciting opportunity to deliver comprehensive primary care, perform advanced clinical assessments, and develop personalized treatment plans for a diverse patient population. The Nurse Practitioner will work closely with solo physician, MA, and support staff to ensure high-quality patient care in a fast-paced environment. Duties Conduct thorough patient assessments, including physical examinations, vital signs, and medical histories to determine health status and needs. Manage acute and chronic conditions across a broad spectrum of internal medicine issues, utilizing evidence-based protocols. Perform procedures as needed utilizing sterile processing techniques with precision and adherence to safety standards. Assist Physician with hospital discharge planning, case management, and utilization review to optimize patient outcomes. Utilize electronic health record (EHR) system for accurate documentation of medical records, medication administration, CPT coding, and clinical notes. Provide telehealth consultations if needed to expand access to care while maintaining HIPAA compliance and ensuring patient privacy. Educate patients on disease prevention, medication management, nutrition, and lifestyle modifications aligned with NCQA standards. Skills Experience with EMR/EHR systems such as Epic or Cerner; proficiency in medical documentation and coding (ICD-10, CPT). Strong clinical skills in internal medicine. Ability to perform minor office procedures as needed. Familiarity with hospital medicine and emergency room protocols. Skilled in patient assessment techniques and physical examinations. Demonstrated ability to work with diverse populations including assisted living residents, nursing home patients, individuals with developmental disabilities or dementia care needs. Strong understanding of medical terminology and physiology knowledge. Excellent communication skills for health coaching, patient education, team collaboration and case management responsibilities. Pay:

$120,000.00 - $155,000.00 per year
