Responsibilities Conduct thorough patient assessments, including vital signs, physical examinations, and medical history reviews to determine appropriate care plans. Manage acute and chronic conditions autonomously Collaborate with interdisciplinary teams to coordinate care plans, discharge planning, and follow-up services for patients in assisted living facilities, nursing homes, or during home visits. Utilize electronic health record (EHR) system to document patient encounters accurately while adhering to HIPAA regulations. Perform diagnostic evaluations using medical imaging or laboratory testing techniques such as sonography or specimen processing to inform treatment decisions. AUTONOMOUS

Nurse Practitioner license with current certification to practice independently in the state Of Florida. Proven experience in diverse clinical settings such as hospital medicine, urgent care, or outpatient clinics; critical care or ICU experience is highly valued. Proficiency with EMR/EHR systems Strong knowledge of medical terminology, physiology, CPT coding, ICD coding Experience working with vulnerable populations including seniors with memory care needs or dementia care patients; familiarity with assisted living or home care environments is preferred. Excellent patient assessment skills combined with the ability to triage effectively in urgent or emergency situations Physical ability to perform duties requiring standing for extended periods and mobility across various locations; valid driver's license preferred for mobile service delivery.
