Job Description

FAMILY NURSE PRACTITIONER
  • PART TIME South Florida Community Health Centers (SFCHC) is launching a new community health center in Pembroke Pines, Florida.
We are seeking a Family Nurse Practitioner to assist in the development of the clinic and provide patient care, initially for two days per week. SFCHC is a nonprofit, sponsored by South Florida Rheumatology (SFR) and Coastal Medical Group. The clinic's mission is to provide primary care services to Pembroke Pines and the surrounding community while supporting chronic disease management for patients referred by SFR. Other services will include preventive care, routine physicals, sick visits, mental health support, and specialist referrals. The Nurse Practitioner will work collaboratively with physicians and specialists affiliated with SFR and Coastal Medical Group and will have access to clinical consultation and support as needed. SFCHC intends to apply for Federally Qualified Health Center Look-Alike (FQHC-LA) designation by August 1, 2026. This is a startup clinic build-out role. Initial patient volumes are expected to be modest as the practice grows. As an early member of the clinical team, you will have the opportunity to help shape clinical workflows, patient experience initiatives, and operational processes.•
RESPONSIBILITIES
Conduct preventive and wellness visits for patients age 16 and older. Diagnose and treat acute and chronic medical conditions. Develop and manage treatment plans. Order and interpret laboratory and diagnostic studies. Provide patient education and counseling. Coordinate specialist referrals and follow-up care. Maintain accurate and timely medical documentation. Participate in clinic workflow development and quality improvement activities.
  • REQUIREMENTS APRN
    degree from an accredited institution. Current, unrestricted Florida APRN license. DEA registration or eligibility. Bilingual English/Spanish required.
  • COMPENSATION
    $50
  • 60 per hour, based on experience. Guaranteed 16 hours per week. Flexible scheduling.
