Position Summary:
We are currently seeking a highly skilled, part-time, Psychiatric Nurse Practitioner to join our mobile care team. In this role, you will provide in‑home healthcare services to patients across our service area, delivering high‑quality care directly where it's needed most. The Mobile NP will assess, diagnose, treat, and manage a wide range of acute and chronic neurological and general medical conditions. This position requires clinical confidence, excellent judgment, and the ability to work independently while maintaining a patient‑centered, relationship‑focused approach. By meeting patients in the comfort of their homes, you will play a vital role in improving access, reducing hospitalizations, and supporting overall quality of life. Key Responsibilities Psych NP Key Responsibilities Diagnose and manage acute and chronic psychiatric conditions such as depression, anxiety disorders, bipolar disorder, schizophrenia, PTSD, and other behavioral health concerns. Conduct comprehensive psychiatric evaluations and mental status examinations in patients' homes, assessing mood, cognition, behavior, risk, and overall functioning. Order and interpret appropriate diagnostic tests , including labs or screenings relevant to psychiatric and general medical care. Prescribe psychiatric medications and develop individualized treatment plans , ensuring safe, evidence‑based management of mental health conditions. Provide therapeutic support and patient education , helping patients and caregivers understand diagnoses, medication use, coping strategies, and ongoing care needs. Document all encounters accurately and promptly in the EMR, maintaining compliance with regulatory and clinical standards. Collaborate with supervising physicians, care coordinators, therapists, and interdisciplinary team members to ensure continuity of care and integrated treatment planning. Perform mobile interventions as appropriate , such as medication administration, injections (e.g., long‑acting antipsychotics), and crisis‑stabilization support within scope. Maintain clinical supplies and uphold infection control standards during home visits. Respond promptly to patient questions, concerns, and follow‑ups , supporting timely, relationship‑centered care. Qualifications Current and unrestricted Nurse Practitioner (NP) license in [State]. Certification as FNP, AGNP, or ANP (based on patient population). Valid DEA license, or application Minimum 1-2 years of NP experience preferred, especially in home health. Ability to work independently in the field with reliable transportation and a valid driver's license. Proficiency with EMR systems and mobile documentation. Strong communication, time management, and interpersonal skills. Preferred Skills & Assets Experience with Medicare, Medicaid, and private insurance documentation.
