Nurse Practitioner Alpha Comfort Care Atlanta, GA Job Details Part-time | Contract $60 an hour 19 hours ago Qualifications Hospice experience Patient assessment BLS Certification Diagnostic evaluation Master's degree Basic life support DEA License Full Job Description Alpha Comfort Care seeks and experience Hospice Nurse Practitioner, to work in community base settings. Position Summary The Nurse Practitioner will provide advanced nursing care to hospice patients in accordance with physician orders, hospice policies, and state and federal regulations. The NP collaborates with the interdisciplinary team to assess, plan, implement, and evaluate care focused on comfort, symptom management, and quality of life for terminally ill patients and their families. The NP serves as a clinical resource to staff and participates in care coordination to ensure patients receive compassionate and appropriate end-of-life care. Essential Duties and Responsibilities Conduct comprehensive face to face patient assessments and develop individualized care plans in person in collaboration with the IDG. Perform face-to-face encounters for hospice recertification in compliance with CMS guidelines. Participate in IDG meeting Manage pain and symptoms associated with terminal illnesses. Collaborate with hospice physicians, nurses, social workers, chaplains, and other interdisciplinary team members. Order, interpret, and evaluate diagnostic tests and treatments as appropriate. Prescribe medications and treatments in accordance with state regulations and collaborative agreements. Educate patients, caregivers, and families regarding disease progression, symptom management, and end-of-life care. Participate in care conferences and interdisciplinary team meetings. Maintain accurate, timely, and compliant clinical documentation within the EMR system. Respond to urgent patient needs and provide clinical guidance to hospice staff. Ensure compliance with Medicare Conditions of Participation, state regulations, and agency policies. Support quality improvement initiatives and regulatory readiness activities. Qualifications Education Master's or Doctorate degree in Nursing from an accredited Nurse Practitioner program. Licensure/Certification Current and unrestricted Nurse Practitioner license in the applicable state. Board Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology NP (AGNP), or related specialty. DEA registration and prescriptive authority required. Current BLS certification required. Experience Minimum of 1-2 years of Nurse Practitioner experience preferred. Hospice, palliative care, geriatrics, oncology, or home care experience strongly preferred. Experience with end-of-life symptom management preferred.
