We are passionate about improving senior health by developing a new care model that centers around the patient, utilizing cutting-edge technology, and expanding access to care in urgent situations to help patients avoid the emergency department and hospitalization.

The Opportunity:

Rosehill Family Care is seeking a Nurse Practitioner to provide care in the Columbus, GA area. The ideal candidate will be passionate about working with a smaller patient panel requiring complex care, and working collaboratively with a motivated multidisciplinary team to provide the best possible care. Our patient-centered approach leverages the expertise of care navigators to maximize the efficiency and effectiveness of the care team.

Expected Tasks:

Provide onsite primary care services to older adults living in Nursing Care Facilities Develop and execute detailed person-centered care plans for primary care patients Work with the team to identify and develop policies to address daily practice and clinical guidelines Work with Medical Assistants & Resident Advocates to provide monthly Chronic Care Management (CCM) visits and Remote Physiologic Monitoring (RPM) Run regular huddles and provide leadership to the clinical team Identify ways to work with senior living management and care teams to better carry out RHFC care plans and improve patient outcomes

What We're Looking For:
