Adult-Gerontology Nurse Practitioner Dublin, GA 31021 $45 an hour - Part-time, Local contract $45 an hour - Part-time, Local contract
OVERVIEW
We are hiring an Adult-Gerontology Nurse Practitioner, who will play a vital role in delivering comprehensive, patient-centered care to ONE older adult in a clinical setting. This position offers an exciting opportunity to utilize your advanced nursing skills, critical thinking, and passion for geriatric and adult health to improve patient outcomes. You will collaborate with multidisciplinary teams, perform detailed assessments, and develop tailored treatment plans that promote health, manage chronic conditions, and enhance quality of life.
GENERAL RESPONSIBILITIES
Conduct thorough physical examinations and patient assessments to evaluate health status across for the geriatric population. Develop and implement individualized care plan, including medication management, health education, and preventive strategies. Manage acute and chronic conditions by utilizing evidence-based practices in areas such as pain management, infectious disease care, gastroenterology, and behavioral health. Provide telehealth consultations to extend care access and coordinate follow-up care through effective triage and case management. Engage in clinical research initiatives or quality improvement projects aimed at advancing adult-gerontology healthcare practices. Collaborate with specialists in fields such as cardiology, nephrology (dialysis), ophthalmology to optimize patient outcomes. Educate patients and their families on disease management, medication administration, nutrition, and lifestyle modifications tailored for aging adults or those with disabilities.
QUALIFICATIONS
Valid Adult-Gerontology Nurse Practitioner certification with current state licensure. Demonstrated experience in critical care environment; hospital medicine or emergency room experience is highly valued. Hands-on experience with procedures like venipuncture (phlebotomy), sterile processing techniques, tube feeding management, wound care, and airway management. Knowledge of complex medical conditions including acute pain management, infectious diseases, hospice & palliative medicine, dementia care (Alzheimer's/memory care), and senior care practices. Experience working in diverse settings such as assisted living facilities, nursing homes (nursing home experience), outpatient clinics, or post-acute care environments. Strong assessment skills in physiology and anatomy. Ability to work effectively in high-pressure situations such as urgent care or trauma scenarios; triage experience is a plus.
